Ontario Medical AI Scribes Found to Produce Dangerous Errors and Hallucinations
An AI scribe listens to a consultation and writes the note that goes in the chart. Ontario's auditor general examined the tools approved for use by doctors in the province and reported that most produced inaccurate, incomplete or fabricated notes, with serious errors found across twenty programmes during procurement, and warned that a wrong note can produce a treatment plan that harms the patient.
